Working collaboratively as part of a multidisciplinary team, the Registered Nurse Case Manager is involved in the case management of clients, including ongoing assessment, determination of unmet need, and care planning in consultation with clients to achieve client goals utilizing appropriate internal and external resources. Case Managers work within a Competency Framework that includes personal and professional accountability, systems thinking, creative thinking, communicating and building relationships. In addition, the Registered Nurse Case Manager is responsible for the provision of direct nursing care to the clients. The Registered Nurse Case Manager ensures that client care is evidence based and client-centered by establishing goals for service delivery and evaluating outcomes.
Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:

Completion of an accredited nursing education program.Active or eligible for registration and practice permit with the College and Association of Registered Nurses of Alberta (CARNA).Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Additional Required Qualifications:

Proven communication and assessment skills to work cooperatively and effectively with the healthcare team members, clients, and their families in a fast paced environment.Strong professional judgment, initiative, and effective time management skills are necessary.Ability to work in different environments within the community (single family homes, lodges, walk up apartments, etc.) performing nursing tasks as well as lift and carry professional bags/equipment weighing up to approximately 25-30 lbs. Preferred Qualifications:

Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in the community sector will be considered.Two years of recent nursing experience (obtained within the last 5 years) or completion of a refresher program within 2 years.Previous Community Care and/or Home Care experience preferred. Experience with Meditech and the RAI Home Care assessment tool is preferred. Please note:
